How to Apply?


To apply onsite, simply visit our school during office hours and proceed to the registration area. Our staff will assist you throughout the application process and guide you in completing the necessary forms.

Before coming to the school, please make sure to prepare all the required documents listed below. Having complete requirements will help speed up your registration.

During your visit, you will be asked to fill out the onsite registration form with accurate and complete information. Once submitted, our team will verify your details along with the documents you’ve provided.

If everything is complete, you may move forward to the next steps of the enrollment process. If you need any help or have questions while applying onsite, our staff will be happy to assist you. Just approach the registration desk, and we will guide you accordingly.

Admission Requirements

  1. Certified true copy of the medical report prepared by any of the following: a developmental doctor, a psychiatrist, or, for related conditions, a medical specialist, as the case may be.
  2. Progress reports from the prospective student’s previous OT, ST, PT, SPED teachers, or ABA therapist, if available.
  3. Assessment reports (direct and indirect assessments) from the previous school.
  4. Certified true copy of the prospective student’s medical certificate, if the parent/ward intends to have him/her participate in strenuous physical activities.
  5. Medical or HMO insurance for each prospective student.
  6. A duly-accomplished School Form which shall be provided by the Academy upon request.
  7. Three (3) copies of the prospective student’s 2x2 ID picture.
